Instructions for Submission:
Please save our Pet Rehabilitation First Visit Form on a computer, complete the form and send to our email, wah@wheatlandah.com. Once you have submitted your First Visit Form, please forward medical records to our email, wah@wheatlandah.com. You may also print out and bring the forms along with any history or records to your first appointment. We look forward to meeting you and your pets.
